微絲（microfilament，MF）又稱肌動蛋白纖維，是普通存在于真核細胞中由肌動蛋白（actin）組成的直徑為5~8nm的螺旋狀纖維。肌動蛋白（actin）是一種中等大小的蛋白質(zhì)，由375-377個氨基酸殘基組成，并且是由一個大的、高度保守的基因編碼。它在生物界有廣泛的分布，除肌肉組織外，還存在于幾乎所有的真核細胞中，如人的腦組織，血小板和多種植物細胞，在很多菌類中也有發(fā)現(xiàn)。

SMA是一種標記平滑肌的肌動蛋白，可以與平滑肌肌動蛋白 異構(gòu)體反應(yīng)，但不能識別來源于成纖維細胞、骨骼肌以及心肌的肌動蛋白。

疾病診斷
診斷標準
WHO專家會建議的診斷原則是：
1、從新鮮糞便標本中查到吞噬有紅細胞的滋養(yǎng)體，或從腸壁活檢組織中查到滋養(yǎng)體是本病確診的可靠依據(jù)。
2、從糞便標本中僅查到1-4個核包囊或腸腔型滋養(yǎng)體，應(yīng)報告為溶組織內(nèi)阿米巴、迪斯帕內(nèi)阿米巴感染。此時即使患者有癥狀，亦不能據(jù)此得出腸阿米巴病的診斷，應(yīng)根據(jù)流行病學(xué)史、血清抗體檢測、糞抗原檢測或PCR檢測證實感染蟲株確屬溶組織內(nèi)阿米巴后，診斷才能確立。否則必須尋找引起腹瀉的其他原因。
3、在有癥狀患者的血清中若能查到高滴度的阿米巴抗體，亦是本病診斷的有力證據(jù)。
輔助檢查
1.糞便檢查：大便呈暗紅色，有糞質(zhì)，帶血、膿或粘液，呈腥臭。
2.乙狀結(jié)腸鏡檢查：如糞檢陰性，乙狀結(jié)腸鏡檢查有很大診斷價值。潰瘍常較表淺，、覆有黃色膿液。潰瘍邊緣略突出，稍見充血，自潰瘍面刮取材料作顯微鏡檢查，發(fā)現(xiàn)病原體的機會較多。
3.阿米巴肝膿腫：腹部B超檢查可見到病灶。
鑒別診斷
應(yīng)與其他原因引起的腸道疾病相鑒別，尤其是細菌性痢疾，尚有腸結(jié)核、血吸蟲病、結(jié)腸炎、結(jié)腸癌及其他腸道原蟲感染亦可與腸阿米巴病混淆。潰瘍病、膽囊病、結(jié)腸息肉等須與有不明確腹痛及腸出血者區(qū)別。綜合運用以上診斷方法，鑒別不難。
疾病治療
1、一般治療：注意休息，進食半流質(zhì)少渣高蛋白飲食。
2、病原治療：甲硝噠唑或稱滅滴靈：原是抗滴蟲藥物，對侵襲組織的阿米巴滋養(yǎng)體有極強的殺滅作用且較安全，適用于腸內(nèi)腸外各型的阿米巴病。劑量為600～800mg、口服、1日3次，連服5～10日；兒童為50mg/kg/日，分3次服，連續(xù)7日。服藥期間偶有抗原抗體、腹痛、頭昏、心慌，不需特殊處理。服藥期忌酒，因可引起精神錯亂。孕婦3個月以內(nèi)及哺乳婦忌用。
疾病預(yù)后
腸阿米巴病及時治療后預(yù)后良好。如并發(fā)腸出血、腸穿孔和彌漫性腹腔炎以及有肝、肺、腦部轉(zhuǎn)移性膿腫者，預(yù)后差。
疾病預(yù)防
煮沸、過濾、消毒飲水，防止吃生菜及防止飲食的被污染，適當處理糞便，防止蒼蠅孳生和滅蠅，均為重要措施。檢查和治療從事飲食業(yè)的排包囊者及慢性患者極為重要。
疾病護理
1.消化道隔離。
2.病情觀察：觀察大便的次數(shù)和形狀、顏色。
3.糞便標本的采集需要注意：及時采集新鮮大便，挑選有粘液、膿血的部分及時送檢，并注意保溫。
